Travel Forms

What forms do I need and where do I find them? All travel forms can be found on gssjc.org in the Forms and Documents section. Please check chapter 7 in Volunteer Essentials for more travel information.

GSSJC Travel Forms:

Trip Forms to Council
1-4 nights within Texas F-413 Short Troop Trip form, roster of all participants**, first aid/CPR certification cards***
5 or more nights in Texas or any trip outside of Texas* F-453 Notice of Intent to Plan, O-883 Initial Plan, O-884 Application for Approval, roster of all participants** and first aid/CPR certification cards***
Any international trip F-453 Notice of Intent to Plan, O-883 Initial Plan, O-884 Application for Approval, roster of all participants** and first aid/CPR certification cards***

*Any trip outside of Texas or more than two nights will require extra insurance to be purchased. The GSSJC manager assisting you with your trip forms can also assist you with obtaining extra insurance. Fees range from 11 cents to $1.17 per person per day, depending on the level of coverage.

**All participants on an overnight trip (girls and adults) should be registered members of GSSJC. All adults should have a background check. Any tag alongs (girls who are younger than 5, boys 12 and younger) must be covered under supplemental insurance, regardless of the trip duration. Boys older than 12 are not allowed to attend overnight activities. Males 18 and older may attend with a membership and background check.

***GSSJC accepts certain certifying organizations for CPR/AED/first aid trainings. Be sure your certification cards meet the requirements before you take the course. At this time, no accepted course in 100 percent online. See chapter 4 of Volunteer Essentials for more information.

Permission slips and medical forms should be kept by the trip leader.
